Individual Treatment Program

If you are at risk for falls and think you need individual treatment, please ask your doctor or health care provider for a referral to Stevens rehabilitation for a physical and occupational therapy evaluation and for treatment for ambulation/mobility and ADLs.

Our individual outpatient program is designed to be very comprehensive, assessing balance, strength, range of motion, safety and independence for daily activities. A home evaluation may be helpful in some cases. Our physical and occupational therapists provide evaluation and treatment for up to 4 to 6 weeks usually once or twice a week, based on the individual’s needs.

Components of our individual treatment program include:

  • Evaluation of range of motion, strength, balance and ability to perform daily tasks safely.
  • Baseline testing.
  • Treatment individualized to meet needs, based on evaluation.
  • Instruction in home programs to improve balance and safety.
  • Education about safety and maintaining good balance during daily tasks.
  • Information about community programs to maintain strength, flexibility and balance.
  • Home evaluations as appropriate.

Please call 425.640.4140 to find out more information.
