Sleep Disorder Questionnaire

Do you suffer from a sleep disorder? If during the past year your answer is "yes" to any of these questions, you may have a sleep disorder:

  • Do you snore?
  • Has anyone told you that you stop breathing during sleep?
  • Do you awaken choking or gasping for breath?
  • Do you feel tired or fatigued during the day?
  • Do you fall asleep before the end of movies or TV programs?
  • Do you have frequent heartburn?
  • Do you have high blood pressure?
  • Do you have uncomfortable feelings in your legs that keep you from falling asleep?
  • Have you been told that your legs jerk or twitch when you are sleeping?
  • Do you often have trouble falling asleep or do you wake up too early?
  • Have you been told that you do unusual things during sleep, such as sleepwalk, act out dreams or shout?
  • When you are waking up or falling asleep, do you feel unable to move?
  • When you're angry or you hear a funny joke, have you experienced loss of muscle control or weakness?
  • Have you gained weight in the past 5 years?
  • Do you feel sleepy while driving?
