Diabetes Support

Today approximately 20 million individuals have diabetes and, unfortunately, two out of three of them will die of cardiovascular disease, namely heart attack and stroke. Stevens Hospital has been recognized by the American Diabetes Association for Quality Self-Management Education for its support of individuals with diabetes. Our program consists of both individual and group classes.

The goal of the program is to provide education so that you can become independent in the daily management of diabetes. We focus on helping you make lifestyle changes to control your blood sugar and decrease your risk for developing complications. The program covers the following areas:

  • Monitoring.
  • Nutrition and meal planning.
  • Exercise.
  • Medications.
  • Sick day management.
  • Dining out.
  • Foot care.
  • Complications and prevention.
  • Management of high/low blood sugars.
  • Community resources.

Participation in the program requires physician referral.

Pre-diabetes/Glucose Intolerance/Metabolic Syndrome

In addition to diabetes, another 20 to 60 million individuals with pre-diabetes and/or metabolic syndrome can delay or prevent the onset of diabetes and cardiovascular complications by making lifestyle changes. Stevens offers individual and group classes focusing on lifestyle management and risk factor modification.
